If you are interested in our services you can print and fill out an application located on the resources page. The files on this page are accessed with a passcode that can be given by request ( just call us at 704-399-4045). The application may be submitted to the administrative staff at The Kid's Workshop. You may also acquire a physical application onsite. You may contact us with any questions on the contact us page.

It is very important that as a parent or guardian you keep your documentation on file here at The Kid's Workshop up to date.This is to ensure we have the most accurate information to be able to get in contact with you in the event of an emergency.

This may include:

• consents
• plan signatures
• authorizations
• emergency contact forms

You will be notified by our administrative team if any of your documentation needs to be updated or signed.
